Before you refer

A useful referral gives the details that matter.

A referral does not need to be long. It should explain who the person is, what is being requested, what matters now and whether there are safety, health, communication or timing details to consider.

Include the participant's suburb, plan context, requested services, preferred contact person, urgency, risks, health needs and communication preferences.
