Privacy and participant information.
Life Focus Care treats personal information, participant details and referral conversations with care, confidentiality and respect.
Why Life Focus Care exists
Life Focus Care started with a simple idea: NDIS services should make the day easier, not harder. Participants, families and coordinators need help that respects each person's routines, preferences and dignity.
Our work is built around ordinary routines. We do not expect someone's day to bend around us. We plan around the way they live, whether that means morning preparation, getting out locally or evening routines.
People should not be left wondering who is arriving or what the plan is. We keep communication steady between participants, families and workers so everyone has the same picture.
Standards for safe, respectful service.
Quality and safety need to show up in the small details: worker screening, privacy, communication and the way concerns are handled.
Trained workers
Workers complete screening, required training and supervision before they support participants.
Privacy respected
Personal information and details about the home are handled with care and confidentiality.
Consistent communication
Families and participants receive useful updates, especially when needs or routines change.
Working with coordinators
We work with support coordinators and allied health professionals when their input is part of the plan.
Participant choice and control
You stay in control of who comes, when they come and how help is provided.
How we work with you
First conversation
It starts with a conversation about the person's routine, preferences and what a good day looks like.
Matching the right worker
We match support workers to the person's needs, routines and communication style.
Review and adjust
Life changes, so services may need to change too. We review what is working and adjust the plan when needed.
