Who we are

Life Focus Care is an RN-owned and operated, registered NDIS provider supporting participants across Melbourne's south-east, the Mornington Peninsula and Gippsland. We provide practical disability and clinical supports at home and in the community.

Our work is person-centred and outcomes-focused. We listen to what matters to each participant, collaborate with families and support networks, and build services around individual routines, communication preferences, health needs and goals.

Our focus is empowering lives through reliable relationships, responsive communication and support that strengthens choice, inclusion and everyday independence.
