Funding and eligibility

Can Life Focus Care help me understand whether my plan can fund support?

We can talk through the support you are seeking, the goals in your plan and the type of funding that may apply. We cannot make funding decisions, but we can help you understand what information may be useful before services begin.

Where can I find NDIS pricing information?

The current NDIS pricing documents are available on our Resources page, including the latest pricing schedule, pricing updates and plain-English guides.

Referrals and service delivery

Who can make a referral?

Participants, family members, support coordinators, health professionals, hospitals, schools, community organisations and GPs can contact us.

What happens after a referral is submitted?

Our intake team reviews the person's location, support needs, urgency, documentation, risks and staff availability before confirming whether Life Focus Care may be the right fit.

Accommodation and respite

What is Supported Independent Living?

Supported Independent Living usually relates to regular assistance with daily tasks in a shared or individual living arrangement. The right model depends on goals, funding, home environment and support needs.

What is Short-Term Accommodation?

Short-Term Accommodation can support planned breaks, short stays, skill building and changes in routine where funding, suitability and availability are in place.

Quality and safety

Do you provide high intensity and nursing supports?

Life Focus Care reviews clinical and high intensity enquiries against staff capability, documentation, training, supervision and governance requirements.

Which areas do you service?

Life Focus Care supports Melbourne's inner and outer south-east, Bayside and Kingston, the Mornington Peninsula, West Gippsland, Bass Coast, Baw Baw, South Gippsland and the Latrobe Valley. Availability depends on the support required and the local team.

Allied health and Support at Home

Can Life Focus Care coordinate allied health services?

Yes. We can coordinate access to trusted allied health clinicians, including physiotherapy, occupational therapy, speech pathology, psychology, exercise physiology, dietetics, podiatry and social work. Availability and funding are confirmed before services begin.

What aged care support is available at home?

Support at Home enquiries may include personal care, domestic help, meals, shopping, transport, social support, respite, nursing, allied health coordination, home safety and practical home-maintenance services.
